Prescription Safety Eyewear

All UK employees (and anyone covered under the UK Discount Program) can receive a discoSunt on the purchase of prescription safety eyewear. Participating Lexington providers of prescription safety eyewear include your local LensCrafters, Pearle Vision and Target Optical.  

1. Review the Authorization Form for pricing and nationwide vendors.
2. Mention discount number 3403208 at your local LensCrafters, Pearle Vision or Target Optical. The form is not required but can be shown to ensure clarity with the vendor. There are a series of options that are available for you to choose from. The discount will be applied at the time of order.
3. You will need a valid eyewear prescription to order safety prescription eyewear (within 1 year). The eye exams are the responsibility of the employee. The doctor of your choice may provide your exam. If you use the UK EyeMed benefit, consult the providers list.
4. If you are required to wear safety eyewear to perform tasks as work, work with your supervisor to complete or update your PPE Hazard Assessment Form. Retain a copy for your records. Submit the form  to Occupational Health and Safety at OHS@uky.edu. or via fax at 859-257-8787.
